Folgard OS (prescription)

I have searched and searched trying to find the exact ingredients in a prescription I started taking one week ago.  I know it has B vitamins, but I don't know WHICH and in WHAT amounts.  I found alot of "Folgard" info, but not on "Folgard OS".  Can anyone pls help?

The OS in folgard OS stands for Osteo, because it has ingredients to help bone health along with the b vitamins and folic acid in the regular formula.
The exact ingredients are: calcium carbonate, magnesium oxide, vitamin D-3, folic acid, b-12(cyanocobalamin), and B-6(pyridoxine)
