Action/Kinetics: A parenteral, semisynthetic antibiotic with an antibacterial spectrum of activity resembling that of carbenicillin. Peak plasma levels: IM, 25-35 mcg/mL after 1 hr; IV, 15 min. t 1/2: 70 min. Elimination complete after 6 hr.

Uses: Primarily suitable for treatment of gram-negative organisms but also effective for mixed infections. (1) Bacterial septicemia, skin and soft tissue infections, acute and chronic respiratory tract infections caused by susceptible strains of Pseudomonas aeruginosa, Proteus, Escherichia coli and other gram-negative organisms. Combined therapy with gentamicin or tobramycin is sometimes indicated for treatment of Pseudomonas infections. (2) GU tract infections (complicated and uncomplicated) caused by the above organisms and by Enterobacter and Streptococcus faecalis. (3) Anaerobic bacteria causing empyema, anaerobic pneumonitis, lung abscess, bacterial septicemia, peritonitis, intra-abdominal abscess, skin and soft tissue infections, salpingitis, endometritis, pelvic inflammatory disease, pelvic abscess. Ticarcillin may be used in infections in which protective mechanisms are impaired such as during use of oncolytic or immunosuppressive drugs or in clients with acute leukemia.

Additional Contraindications: Pregnancy.

Special Concerns: Use with caution in presence of impaired renal function and for clients on restricted salt diets.

Additional Side Effects: Neurotoxicity and neuromuscular excitability, especially in clients with impaired renal function.

Laboratory Test Alterations: Alkaline phosphatase, AST, ALT.

Additional Drug Interactions: Effect of carbenicillin may be enhanced when used in combination with gentamicin or tobramycin for Pseudomonas infections.

How Supplied: Powder for injection: 3 g, 20 g

Dosage
?IV Infusion, Direct IV, IM Bacterial septicemia, intra-abdominal infections, skin and soft tissue infections, infections of the female genital system and pelvis, respiratory tract infections.
Adults: 200-300 mg/kg/day by IV infusion in divided doses q 4 or 6 hr (3 g q 4 hr or 4 g q 6 hr), depending on the weight and severity of the infection. Pediatric, less than 40 kg: 200-300 mg/kg/day by IV infusion q 4 or 6 hr (daily dose should not exceed the adult dose).
UTIs, uncomplicated.
Adults: 1 g IM or direct IV q 6 hr. Pediatric, less than 40 kg: 50-100 mg/kg/day IM or direct IV in divided doses q 6 or 8 hr.
UTIs, complicated.
Adults: 150-200 mg/kg/day by IV infusion in divided doses q 4 or 6 hr (usual dose is 3 g q.i.d. for a 70-kg client).
Neonates with sepsis due to Pseudomonas, Proteus, or E. coli.
Less than 7 days of age and less than 2 kg, 75 mg/kg q 12 hr; more than 7 days of age and less than 2 kg, 75 mg/kg q 8 hr; less than 7 days of age and more than 2 kg, 75 mg/kg q 8 hr; more than 7 days of age and more than 2 kg, 100 mg/kg q 8 hr. Can be given IM or by IV infusion over 10-20 min.
Clients with renal insufficiency should receive a loading dose of 3 g IV, and subsequent doses, as follows by C CR. C CR over 60 mL/min: 3 g IV q 4 hr; C CR from 30-60 mL/min: 2 g IV q 4 hr; C CR from 10-30 mL/min: 2 g IV q 8 hr; C CR, less than 10 mL/min: 2 g IV q 12 hr or 1 g IM q 6 hr; C CR, less than 10 mL/min with hepatic dysfunction: 2 g IV q 24 hr or 1 g IM q 12 hr. Clients on peritoneal dialysis: 3 g IV q 12 hr; clients on hemodialysis: 2 g IV q 12 hr and 3 g after each dialysis.
