Classification: Cephalosporin, first-generation

Action/Kinetics: Peak serum concentration: IM 17-76 mcg/mL after 1 hr. t 1/2: IM, IV: 90-120 min. From 60% to 80% excreted unchanged in urine.

Uses: (1) Respiratory tract infections due to Streptococcus pneumoniae, Klebsiella, Haemophilus influenzae, Staphylococcus aureus and group A ß-hemolytic streptococci. (2) GU infections (prostatitis, epididymitis) due to Escherichia coli, Proteus mirabilis, Klebsiella, and some strains of Enterobacter and enterococci. (3) Skin and skin structure infections due to S. aureus, group A ß-hemolytic streptococci, and other strains of streptococci. (4) Biliary tract infections due to E. coli various strains of streptococci, P. mirabilis, Klebsiella, and S. aureus. (5) Bone and joint infections due to S. aureus. (6) Septicemia due to S. pneumoniae, S. aureus, P. mirabilis, E. coli, and Klebsiella. (7) Endocarditis due to S. aureus and group A ß-hemolytic streptococci. (8) Perioperative prophylaxis may reduce the incidence of certain postoperative infections in those having contaminated or potentially contaminated surgical procedures (e.g., vaginal hysterectomy or cholescystectomy).

Special Concerns: Safety in infants under 1 month of age has not been determined.

Additional Side Effects: When high doses are used in renal failure clients: extreme confusion, tonic-clonic seizures mild hemiparesis.

How Supplied: Injection: 1 g/50 mL, 500 mg/50 mL; Powder for injection: 500 mg, 1 g, 10 g, 20 g

Dosage
?IM, IV Only Mild infections due to gram-positive cocci.
Adults: 250-500 mg q 8 hr.
Mild to moderate infections.
Children over 1 month: 25-50 mg/kg/day in three to four doses.
Moderate to severe infections.
Adults: 0.5-1 g q 6-8 hr.
Acute, uncomplicated UTIs.
Adults: 1 g q 12 hr. For severe infections up to 100 mg/kg/day may be used.
Endocarditis, septicemia.
Adults: 1-1.5 g q 6 hr (rarely, up to 12 g/day).
Pneumococcal pneumonia.
Adults: 0.5 g q 12 hr.
Preoperative prophylaxis.
Adults: 1 g 30-60 min prior to surgery.
During surgery of 2 or more hours.
Adults: 0.5-1 g.
Postoperative prophylaxis.
Adults: 0.5-1 g q 6-8 hr for 24 hr (may be given for 3-5 days, especially in open heart surgery or prosthetic arthroplasty).
Impaired renal function.
Initial: 0.5 g; then, maintenance doses are given, depending on C CR, according to schedule provided by manufacturer.
