Classification: Narcotic agonist/antagonist

Action/Kinetics: Semisynthetic opiate possessing both narcotic agonist and antagonist activity. It has limited activity at the mu receptor. IM, onset: 15 min; Peak effect: 1 hr; Duration: 6 hr. t 1/2: 2-3 hr. May also be given IV with shorter onset and peak effect. Is about equipotent with naloxone as a narcotic antagonist.

Uses: Moderate to severe pain.

Special Concerns: Use during lactation only if benefits outweigh risks. Use in children less than 2 years of age has not been established. Use with caution in clients with compromised respiratory function, in head injuries, in impairment of liver or renal function, Addison's disease, prostatic hypertrophy, biliary tract dysfunction, urethral stricture, myxedema, and hypothyroidism. Administration to individuals physically dependent on narcotics may result in precipitation of a withdrawal syndrome.

Side Effects: CNS: Sedation, dizziness, confusion, headache, euphoria, slurred speech, depression, paresthesia, psychosis, malaise, hallucinations, coma, dysphoria, agitation, seizures. GI: N&V;, constipation, dyspepsia, loss of appetite, dry mouth. Ophthalmologic: Miosis, blurred vision, double vision, conjunctivitis. CV: Hypotension, bradycardia, tachycardia, Wenckebach block. Respiratory: Decreased respiratory rate, cyanosis, dyspepsia. Dermatologic: Sweating, rash, pruritus, flushing. Other: Urinary retention, chills, tinnitus.

Drug Interactions: Additive CNS depression with alcohol, general anesthetics, antianxiety agents, sedative-hypnotics, phenothiazines, and other narcotic analgesics.

How Supplied: Injection: Equivalent to 0.3 mg/mL buprenorphine

Dosage
?IM, Slow IV Analgesia.
Over 13 years of age: 0.3 mg q 6 hr. Up to 0.6 mg may be given; doses greater than 0.6 mg not recommended. Children, 2-12 years of age: 2-6 mcg/kg q 4-6 hr. Do not give single doses greater than 6 mcg/kg.
