Have you ever joined a weight loss treatment or visited a weight loss clinic where the counselor is a dietitian or a physician (or any other health professional)? Have you ever felt satisfied or dissatisfied with the behaviors of the practitioners counseling you for weight loss?

Here is your chance to share with us your opinions and experiences with your practitioners’ behaviors during your weight-loss treatment.

The information you will provide in this survey will be used for the improvement of weight loss clinics. Any false information will affect the treatment of hundreds of people. For that reason, please participate only if you’ve had the experience we’re asking about.

This survey will take about 15 minutes of your time and is anonymous.

As a thank you for your participation, you will enter a raffle with the chance of winning $100.

You will be able to participate if you are between the ages of 20 and 65, and you think you are overweight with no morbid health conditions or diagnosed eating disorders.

If you wish to participate, please log on to http://dons.usfca.edu/swabdelnour/patients.htm You will find on that webpage a link to access an Information Sheet, for more information about this research; the Research Subject’s Bill of Rights; and a link to the questionnaire Dietetics Professionals’ Practices–Patients, which you will complete.

Please complete the questionnaire by May 15, 2005.

Please address any concerns regarding the study or the questionnaire to the researcher.
