Action/Kinetics:
An antimitotic agent that causes necrosis of visible wart tissue when applied topically. Small amounts are absorbed into the system 1-2 hr after application. t1/2: 1-4.5 hr. The drug does not accumulate following multiple treatments.
Uses:
Topical treatment of external genital warts and perianal warts (gel only). Investigational: Systemically for treatment of cancer.
Contraindications:
Use of solution or gel for mucous membrane warts or solution for perianal warts. Lactation.
Special Concerns:
Genital warts must be distinguished from squamous cell carcinoma prior to initiation of treatment. Safety and effectiveness have not been demonstrated in children. Avoid contact with the eyes.
Side Effects:
Solution. Dermatologic: Commonly, burning, pain, inflammation, erosion, and itching. Also, tenderness, chafing, scarring, vesicle formation, dryness and peeling, tingling, bleeding, ulceration, malodor, crusting edema, foreskin irretraction. Miscellaneous: Pain with intercourse, insomnia, dizziness, hematuria, vomiting.
Gel. Dermatologic: Commonly, burning, pain, inflammation, erosion, itching, and bleeding. Also, stinging, erythema, desquamation, scabbing, discoloration, tenderness, dryness, crusting, fissures, soreness, ulceration, swelling/edema, tingling, rash, blisters.
Systemic Use. GI: N&V;, diarrhea, oral ulcers. Hematologic: Bone marrow depression, leukocytosis, pancytosis. CNS: Altered mental status, lethargy, coma, seizures. Miscellaneous: Peripheral neuropathy, fever, tachypnea, respiratory failure hematuria, renal failure.
Overdose Management:
Symptoms: N&V;, diarrhea, fever, altered mental status, hematologic toxicity, peripheral neuropathy, lethargy, tachypnea, respiratory failure hematuria, leukocytosis, pancytosis, renal failure, seizures, coma. Treatment: Wash the skin free of any remaining drug. General supportive therapy to treat symptoms.

Dosage
•Topical Solution, Topical Gel
External genital warts, perianal warts (gel only).
Adults, initial: Apply b.i.d. in the morning and evening (i.e., q 12 hr) for 3 consecutive days; then, withhold use for 4 consecutive days. The 1-week cycle of treatment may be repeated up to 4 times until there is no visible sign of wart tissue. Consider alternative treatment if the response is incomplete after four treatments. |
