Action/Kinetics:
Bactericidal drug that inactivates enzyme enolpyruvyl transferase, irreversibly blocking condensation of uridine diphosphate-N-acetylglucosamine with p-enolpyruvate. This is one of first steps in bacterial wall synthesis. Also reduces adherence of bacteria to uroepithelial cells. Rapidly absorbed from GI tract and converted to fosfomycin. Maximum serum levels: 2 hr. t1/2, elimination: 5.7 hr. Excreted unchanged in both urine and feces.
Uses:
Treatment of uncomplicated urinary tract infections (acute cystitis) in women due to Escherichia coli and Enterococcus faecalis.
Contraindications:
Lactation.
Special Concerns:
Safety and efficacy have not been determined in children 12 years and younger.
Side Effects:
GI: Diarrhea, nausea, dyspepsia, abdominal pain, abnormal stools, anorexia, constipation, dry mouth, flatulence, vomiting. CNS: Headache, dizziness, insomnia, migraine, nervousness, paresthesia, somnolence. GU: Vaginitis, dysmenorrhea, dysuria, hematuria, menstrual disorder. Respiratory: Rhinitis, pharyngitis. Miscellaneous: Asthenia, back pain, pain, rash, ear disorder, fever, flu syndrome, infection, lymphadenopathy, myalgia, pruritus, skin disorder.
Laboratory Test Alterations:
ALT, AST, eosinophil count, bilirubin, alkaline phosphatase. Hematocrit, hemoglobin. or WBC, platelet count.
Drug Interactions:
- Metoclopramide /
serum levels and urinary excretion of fosfomycin.

Women, 18 years and older: One sachet of fosfomycin mixed with water before ingesting. |
