Action/Kinetics: Derived from kanamycin. Its spectrum is somewhat broader than that of other aminoglycosides, including Serratia and Acinetobacter species, as well as certain staphylococci and streptococci. Effective against both penicillinase- and non-penicillinase-producing organisms. Peak therapeutic serum levels: IM, 16-32 mcg/mL. t1/2: 2-3 hr. Toxic serum levels: >35 mcg/mL (peak measured after 1 hr) and >10 mcg/mL (trough measured before next dose).

Uses: Short-term treatment of gram-negative bacterial infections including Pseudomonas, Escherichia coli, Proteus, Providencia, Klebsiella, Enterobacter, Serratia and Acinetobacter. For infections due to gentamicin or tobramycin resistant strains of Providencia rettgeri, P. stuartii, Serratia marcescens and Pseudomonas aeruginosa.
Infections include bacterial septicemia (including neonatal sepsis); serious infections of the respiratory tract, bones, joints, skin, soft tissue, and CNS (including meningitis); intra-abdominal infections (including peritonitis); burns; postoperative infections (including postvascular surgery). Also, serious complicated and recurrent infections of the urinary tract. May be used as initial therapy in certain situations in the treatment of known or suspected staphylococcal disease. Investigational: Intrathecal or intraventricular use. As part of multiple drug regimen for Mycobacterium avium complex (commonly seen in AIDS clients).

Special Concerns: Use with caution in premature infants and neonates.

How Supplied: Injection: 5 mg/mL, 50 mg/mL, 62.5 mg/mL, 250 mg/mL

Dosage
•IM (Preferred) and IV
Adults, children, and older infants: 15 mg/kg/day in two to three equally divided doses q 8-12 hr for 7-10 days; maximum daily dose: 15 mg/kg.
Uncomplicated UTIs.
250 mg b.i.d.; newborns: loading dose of 10 mg/kg followed by 7.5 mg/kg q 12 hr.
Use in neonates.
Initial: Loading dose of 10 mg/kg; then, 7.5 mg/kg q 12 hr. Lower doses may be safer during the first 2 weeks of life.
Intrathecal or intraventricular use.
8 mg/24 hr.
As part of multiple drug regimen for M. avium complex.
15 mg/kg/day IV in divided doses q 8-12 hr.
In clients with impaired renal function.
Normal loading dose of 7.5 mg/kg; then monitor administration by serum level of amikacin (35 mcg/mL maximum) or creatinine clearance rates. Duration of treatment: Usual: 7-10 days.
